Special Authorisation

I hereby voluntarily authorise Stop Gambling SA (operating the website stopgambling.co.za) and any person duly authorised by Stop Gambling SA to assist me by acting as my representative solely for the limited purposes described below, in relation to my voluntary cool-off period request from selected online gambling operators.
This authorisation is limited to facilitation and assistance and does not grant Stop Gambling SA any independent legal authority beyond what I explicitly provide here.
Authorised Actions (limited to the online gambling operators I specifically listed in my cool-off period request):

1. To prepare, complete, and submit (where permitted by the recipient) cool-off period application forms, requests, emails or supporting documents on my behalf.

2. To provide my personal information, including but not limited to my full name, ID/passport number, date of birth, address, contact details, and any other information I supply or is required, solely for the purpose of processing my voluntary cool-off period request.

3. To communicate with the specified online gambling operators on my behalf regarding my cool-off period request, including responding to reasonable queries, forwarding additional documentation I provide, and seeking confirmation of the cool-off period implementation where possible.

4. To take other reasonable administrative steps to support my cool-off period request, such as drafting and sending correspondence or emails to the specified online gambling operators.

Scope and Duration:

• This authorisation is limited to the licensed gambling venues, operators, and platforms in South Africa that I specifically identify and request in my online cool-off period request completed via the stopgambling.co.za website. Stop Gambling SA retains a copy of my submitted request for reference and inspection if needed.

• The intended cool-off period is as specified with the entities I list in my online request.

• This authorisation takes effect immediately upon my electronic/digital signature and remains in force until I revoke it in writing or until my cool-off period request has been fully processed (or determined to be unsuccessful) by the relevant online gambling operators, whichever occurs first.

• I understand and accept that this cool-off period request is a voluntary request and specifically state that I am not a gambling addict and that I will seek further assistance should my recreational gambling become an addiction, and that ultimate responsibility for gambling responsibly rests with me.

Important Declarations, Acknowledgments, and Waivers:

1. I confirm that I am providing this authorisation freely, voluntarily, and without any undue influence. I am of sound mind, fully understand the implications of a cool-off period (including that my account with the online gambling operators will be locked and/or closed), and am acting in my own best interest.

2. I hereby release, indemnify, and hold harmless Stop Gambling SA, its operators, website administrators, any authorised persons assisting through it, and all related parties from any and all liability, claims, losses, damages, or consequences arising from or related to:

• Actions or assistance provided under this authorisation (including any errors, omissions, delays, incomplete submissions, or technical issues);

• Non-acceptance, rejection, or partial implementation of my cool-off period request by online gambling operators;

• Any direct or indirect outcomes of my cool-off period request.

• This release applies except in cases of proven intentional misconduct or gross negligence by Stop Gambling SA.

3. I consent to the collection, processing, and sharing of my personal information by Stop Gambling SA (and any persons assisting it) strictly in accordance with the Protection of Personal Information Act (POPIA), and only to the extent necessary to facilitate my cool-off period request as described above. I understand that such sharing is limited to the online gambling operators I specify in my request.

4. I acknowledge that some online gambling operators may require direct personal confirmation or other verification steps. This authorisation and any submissions made via Stop Gambling SA may not be accepted by some online gambling operators. Stop Gambling SA will inform me of any known rejections or issues of which it becomes aware, but I accept full responsibility for following up directly with any online gambling operators that do not accept third-party assistance.

5. In support of my request, I agree to provide (upload or otherwise supply):
(a) A copy of my ID or passport;
(b) A recent photograph of myself;
(c) Proof of address (if requested or available).

Revocation: I may revoke this authorisation at any time by sending written notice (e.g., email) to Stop Gambling SA at info@stopgambling.co.za mailto:info@stopgambling.co.za). Revocation will not affect any actions already completed or submitted before receipt of the revocation notice.
Governing Law: This authorisation is governed by the laws of the Republic of South Africa.
Electronic/Digital Signature: By electronically signing below (or confirming via the online process on stopgambling.co.za), I confirm my identity, my intent to be bound by these terms, and that this constitutes my valid signature.
